Documentation required from players by the TOC

To provide for a seamless check-in to the tournament, please make sure that the following items have been taken care of:

1. Your personal information has been entered correctly into the on-line registration system, your photograph has been uploaded and accommodation and meal options have been selected for you.  We also need to know the jersey numbers of all ultimate players in advance and we ask that you don't change those numbers during the course of the tournament.
2. You have made sure that you have medical insurance that covers you for tournament play.  It is possible to sign up for medical insurance through the on-line registration system. More details are available within the system. If you are covered by your own medical insurance and you would like to receive priority treatment at a private medical clinic in Turku, please mail us a document indicating proof of coverage by July 19th.
3. You have downloaded, filled in and signed the following waiver and handed it to your country contact person for mailing by July 19th at the latest to the following address:
